Need help navigating healthcare, housing, benefits, or appointments?

ECM provides a dedicated Lead Care Manager to help coordinate services and make sure you get the support you need. Join ECM for the following benefits:

• No Cost: ECM is a no-cost benefit included with your Medi-Cal plan.
• Dedicated Support: You will be provided with a Lead Care Manager who works with you one-on-one.
• Care Coordination: We work with your doctors, specialists, pharmacists, and others to get your needs met.
• Your Choice: Joining ECM is a choice. You can leave the program at any time and keep your Medi-Cal benefits.

• Transportation Assistance: Set up rides to doctor visits and appointments.
• Whole-Person Care: Plan for your physical, mental, and dental needs.
• Healthcare Coordination: Connect you with doctors and make appointments.

• Medication Support: Help you better understand your medications.
